    This form must be filled out for all journeyman and apprentice trades/crafts that will be used on the public works project. Please note the following when filling out the Training Fung section of the Form:
     (1) Apprenticeable Trade/Craft    When journeymen and apprentices are employed on the public works project, the training fund contribution monies must be paid to the applicable Apprenticeship     Committee or the California Apprenticeship Council.
    (2) Non-Apprenticeable Trade/Craft
    The training fund contribution monies are paid directly to the employee when the trade/craft is not apprenticeable.
  • Certified Payroll Records
    Per Labor Code Section 1776, contractors and subcontractors are required to keep accurate payroll records which reflect the name, address social security number, and work classification of each employee; the straight time and overtime hours worked each day and each week; the fringe benefits; and the actual per diem wages paid to each journeymen, apprentice, worker, or other employee hired in connection with a public works project... 
    Contractors and subcontractors shall maintain their certified payrolls on a weekly basis and shall submit said payrolls weekly to: SMC Construction Co., 12 Mauchly, Suite D, Irvine CA 92618.
  • FINAL Certified Payroll form
    This form is filled out same as above but submitted only upon the FINAL payroll week ending.
  • Statement of Non-Performance
    In the event that there has been no work performed during a given week, this form can be used or the Certified Payroll Form shall be annotated "NO WORK" for that week.

  • Public Works Award Contract Information Form (DAS140)
    Contractors who are not already approved to train by an apprenticeship program sponsor shall provide contract award information to all of the applicable apprenticeship committees whose geographic area of operation includes the area of the public works project. Submit contract award information to the apprenticeship committee for each apprenticeable craft or trade in the area of the site of the public works project that has approved the contractors, who are participants in an approved apprenticeship program, to train apprentices. (Title 8, California Code of Regulations, Section 230).
    The contract award information shall be in writing and may be on a Public Works Contract Award Information form (DAS 140). The information shall be provided to the applicable apprenticeship committee within ten (10) days of the date of the execution of the prime contract or subcontract, but in no event later than the first day in which the contractor has workers employed upon the public work. (Title 8, California Code of Regulations, Section 230). The filing of a DAS 140 is not a request for dispatch of registered apprentices.

  • Request for Dispatch of An Apprentice (DAS142)
    Employ registered apprentices on the Public Works project in a ratio of no less than one (1) hour of apprentice work for every five (5) hours performed by a journeyman. (Title 8, California Code of Regulations, Section 230.1)
    All contractors must request dispatch of required apprentices from an Apprenticeship Program (for each apprenticeable craft or trade) by giving the Program actual notice of at least 48 hours (excluding Saturdays, Sundays and holidays) before the date on which apprentices are required. Contractors who are not already participating in an approved program and who did not receive sufficient number of apprentices from their initial request, must request dispatch of apprentices from at least one other Apprenticeship Committee, if more than one exists in the area of the public works project. (Title 8, California Code of Regulations, Section 230.1(a))

  • Training Fund Contributions Form (CAC2)
    Make training fund contributions in the amount established in the Prevailing Wage Rate publication for journeymen and apprentices. Contractors who contribute to an apprenticeship program are entitled to a full credit in the amount of those contributions. Contractors who do not contribute to an apprenticeship program must submit their contributions to the California Apprenticeship Council, P. O. Box 420603, San Francisco, CA 94142-0603. Training fund contributions to the Council are due and payable on the 15th day of the month for work performed during the preceding month.
    Training contribution to the Council shall be paid by check and shall be accompanied by a completed Training Fund Contributions form (CAC-2), or a form containing the following information (Title 8, California Code of Regulations, Section 230.2(c)):
    (1) The name, address, and telephone number of the contractor making the contribution.
    (2) The contractor's license number.
    (3) The name and address of the public agency that awarded the contract.
    (4) The jobsite location, including the county where the work was performed.
    (5) The contract or project number.
    (6) The time period covered by the enclosed contributions.
    (7) The contribution rate and total hours worked by apprenticeable occupation.
    (8) The name of the program(s) that provided apprentices, if any.
    (9) The number of apprentice hours worked, by apprenticeable occupation and by program.
Mechanics Lien Releases
  • Conditional Waiver & Release upon Progress Payment
    Use this form when the claimant is required to execute a waiver and release in exchange for or in order to induce the payment of a progress payment and the claimant has not been paid. This form is useful when the claimant has not been paid yet, but will be paid out of a progress payment that is not the final payment. This conditional waiver and release is only effective if the claimant is actually paid. This release does not cover all items. See the "Conditional Waiver And Release Upon Progress Payment" form for more information.
  • Unconditional Waiver & Release upon Progress Payment
    Use this form when the claimant is required to execute a waiver and release in exchange for or in order to induce payment of a progress payment and the claimant asserts in the waiver that he or she has in fact been paid the progress payment. This release does not cover all items. See the "Unconditional Waiver And Release Upon Progress Payment" form for more information.
  • Conditional Waiver & Release upon Final Payment
    Use this form when the claimant is required to execute a waiver and release in exchange for or in order to induce the payment of a final payment and the claimant has not been paid. This release is only binding if there is evidence of payment to the claimant. Evidence of payment may be demonstrated by:
    • the claimant's endorsement on a single check or a joint payee check which has been paid by the bank upon which it was drawn; or
    • written acknowledgment of payment given by the claimant.

  • Unconditional Waiver & Release upon Final Payment
    Use this form when the claimant is required to execute a waiver and release in exchange for, or in order to induce payment of, a final payment and the claimant asserts in the waiver he or she has in fact been paid the final payment.
